REGISTRATION CONSENT/WAIVER By registering for this class or activity and attending (in person or electronically) you give your consent to YogaRoots On Location, LLC (“Presenter”) to record your participation in the event or program described. You understand that the presentation/or participation therein may be videotaped, audiotaped, photographed, recorded, edited or otherwise reproduced for use for commercial or non-commercial purposes by Presenter, including, but not limited to books (including electronic and audio versions), instructional settings, Internet uses (including webcasts and podcasts and streaming video), or through any other communications media currently existing or later created. You understand, however, that Presenter agrees, warrants, and represents that it will anonymize any participation and your name, likeness, or unique identifiers will not be published, reproduced, or distributed for any of the uses described above, except for internal training purposes for Presenter’s staff. You waive all rights to damages, claims, or causes of action in relation to the recording of the class or activity absent a breach of this consent language by Presenter. You further understand that any and all content, curriculum, methods, images, and presentations used in the class or activity are either the property of Presenter or works for which Presenter has the permission of the intellectual property owner to use in this way. Therefore, you agree that you will not record, photograph, distribute, redistribute, offer for sale, modify, share, extrapolate from, or otherwise use any of the content of the class or activity without express written consent of Presenter. This class may only be used by participants for its intended personal use. AGREEMENT OF RELEASE AND WAIVER OF LIABILITY PLEASE INFORM THE INSTRUCTOR AT THE BEGINNING OF CLASS IF YOU HAVE ANY INJURIES OR LIMITATIONS. By checking the box below, I intend to be legally bound by the following: I warrant and represent that I am in good physical health and do not suffer from any medical condition which would limit my participation in the yoga classes, workshops, or other activities offered at YogaRoots On Location, LLC or that such activities would endanger my health. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in any of the classes, workshops, or other activities offered by YogaRoots On Location, LLC. I understand that yoga is not a substitute for medical attention, examination, diagnosis or treatment. I represent and warrant that I understand the potential risks and hazards associated with the physical activities offered by YogaRoots on Location, LLC and I agree to follow all instructions so that I may safely participate in classes, workshops, or other activities. If I experience any pain or discomfort, I will listen to my body, ask for support from the teacher, and/or discontinue all activity. I hereby WAIVE AND RELEASE YogaRoots On Location, LLC, and its owners, officers, employees, contractors, and instructors from any claim, demand, or cause of action of any kind resulting from or related to my participation in the programs offered by YogaRoots On Location, LLC or any location owned or rented by YogaRoots On Location, LLC. In taking part in the yoga classes, workshops, or other activities offered by YogaRoots On Location, LLC I understand and acknowledge that I am fully responsible and liable for all risks, injuries, or damages, known or unknown, which might occur as a result of my participation in the classes, workshops, or other activities. Furthermore, I accept full liability and responsibility for any injury that I may cause either to myself or to any other participant and/or instructor while in attendance at an activity or class offered by YogaRoots On Location, LLC or at YogaRoots On Location, LLC’s studio. I agree to INDEMNIFY AND HOLD HARMLESS YogaRoots On Location, LLC, and its owners, officers, employees, contractors, and instructors from any and all liability for the injury of myself and/or the injury or death of any other person(s) and/or damage to property that may result from my actions while participating in activities offered by YogaRoots On Location, LLC. I have read the above release and waiver of liability and fully understand its content. I am legally competent to sign and voluntarily agree to the terms and conditions stated above. This agreement is governed by the laws of the State of Pennsylvania.
